National Diabetes Prevention Program

Did you know that 1 out of 3 adults with prediabetes are not aware that they are at risk? Don’t wait!

We have partnered with the American Pharmacists Association (APhA) to provide a complimentary lifestyle change program to help prevent type 2 diabetes and promote healthy living. This program is funded by the Centers for Disease Control and Prevention (CDC) through an award from the APhA Foundation.

Take control of your health to prevent diabetes

The National Diabetes Prevention Program offers participants on-demand access to telehealth sessions facilitated by lifestyle coaches.

What can be expected of the program?

  • Convenience and confidentiality
  • Helpful tips for healthy living
  • Reduction of risks associated with type 2 diabetes
  • A personal lifestyle coach

Who is eligible to participate in the program?

  1. Adults (18 years or older)
  2. Not pregnant at time of enrollment​
  3. BMI ≥ 25 (≥ 23 if Asian)
  4. Have no previous diagnoses of type 1 or type 2 diabetes
  5. Qualify for the program based on either:
    • A blood test result in the prediabetes range within the past year via ONE of the following:
      • Hemoglobin A1C: 5.7%–6.4%
      • Fasting glucose: 100–125 mg/dL (or 110–125 if Medicare recipient)
      • Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL
      • Clinical diagnosis of gestational diabetes during previous pregnancy
    • CDC/American Diabetes Association Prediabetes Screening Test score of 5 or higher
